    Death record applications will be accepted at the Division of Property Records and Taxation (Public Service Counter), 2100 3rd Ave., Anoka, MN 55303 and all License Centers.

     

    Deceased 1997 and After

    A certified death record for a death in Minnesota in 1997 and after can be purchased at the Division of Property Records and Taxation (Public Service Counter) office and at all License Centers.

    Deceased Before 1997

    A certified death record for a death recorded in Anoka County before 1997 can only be obtained from the Division of Property Records and Taxation (Public Service Counter).

    In order to obtain a certified death record, parties must show that they have a tangible interest in the record.

    Required Forms

    For death records available through Anoka County, complete the application process by clicking on the Next button. Applications can also be picked up at the Division of Property Records and Taxation (Public Service Counter) or at any License Center or an application form can be sent to you.

    Parties need to submit a completed application form, the required identification and the required fee. In some cases it may also be necessary to submit a Statement to Identify Certified Birth or Death Certificate Applicant.

    Required Identification

    Any of the following documents are acceptable unless the document has been altered or changed, or there is evidence of fraud on the face of the document:

    • Unexpired picture driver's license of a U.S. state or territory
    • Unexpired picture identification card of a U.S. state or territory
    • Unexpired U.S. federal, state, or local government employee picture ID
    • Unexpired FAA aircraft pilot's license
    • Unexpired U.S. military picture ID
    • Unexpired U.S. passport

    Required Fees

    • $13.00 First Certified Death Record without cause of death (only for records 1997 to present)
    • $13.00 First Certified Death Record with cause of death
    • $6.00 Each additional Death Record of the same record issued at the same time
